FTM Top Surgery

Overview

FTM top surgery is an important step for transitioning patients. It allows them to feel more aligned with their gender identity and thus more authentic in their interactions and presentation to the world. At FPSG, we are honored to assist transmasculine and nonbinary patients along their journey toward achieving their ideal physique. The surgery involves the removal of breast tissue and the reshaping of the chest to accomplish a more chiseled, sculpted upper body profile. 

Procedure

With FTM top surgery, one of two incision types is selected. For those with less breast tissue, a periareolar incision can be used. This incision wraps around the nipple, with scarring well hidden within the darker pigments of the areola. The second type of incision pattern is more common for patients with a moderate to significant amount of breast tissue to be removed. Two incisions are created, one horizontally along the inframammary fold and one vertically down the chest center. Your surgeon may also choose to utilize a keyhole incision pattern or an anchor incision pattern. Our surgeons will choose the best approach for your body and goals.

Recovery

After top surgery, a specialized dressing will need to remain in place for 5-7 days to protect the nipple graft. Some patients may be fitted with drainage tubes to escort any excess fluids and blood out of the body. Pain can be well-controlled with medications prescribed by your plastic surgeon. You should limit arm usage for the first 7 days, which includes lifting (including picking up children and pets) and driving. Regular exercise and weight-lifting can be resumed within around 3-6 weeks, depending on your provider’s instructions. Scarring will gradually fade over the course of the next year. MTF Top Surgery

Overview

MTF top surgery is a gender-affirming procedure that can help female-identified patients to enjoy enhanced cleavage, a wider array of wardrobe options, and a significant boost in self-confidence. The goal of the surgery is to create natural-looking breasts that complement the patient’s figure and bring balance and harmony to the silhouette. Our experienced surgeons can help you to decide on the size and shape of your implants. Breast enhancement with fat transfer is also an option.

Procedure

While estrogen hormone therapy can help stimulate breast growth, many transfeminine patients do not feel their transition is complete and may wish to undergo top surgery with implants or fat transfer. In the case of the latter, fat cells are extracted from the belly, thighs, or hips and then injected into the chest to create a modest increase in cleavage. With implant surgery, your surgeon will create a capsule in the chest to hold the implant in place before inserting a silicone or saline device through an incision of your choice (around the areola, within the armpit, or through the inframammary fold.) If hormone therapy has not produced sufficient breast tissue to house the implants, a tissue expander can be used to stretch the chest skin until the patient is ready for top surgery. 

Recovery

After top surgery, swelling, bruising, and soreness in the chest are common symptoms. Your surgeon will provide you with prescription pain medications and a personalized aftercare plan to help make the healing journey more comfortable. The chest should be wrapped in a compression garment to help reduce swelling and keep the newly sculpted contours in perfect shape. Most patients can expect to resume work and normal activities within 2 weeks. However, it’s critical that you avoid strenuous exercise and heavy lifting for at least 4-6 weeks.
